Some of you requested this option so you could add the patient to your queue soon after admission but without yet attempting to contact the patient. Once you attempt to contact the patient, you will need to change the status to either: Open First Attempt Made if you attempted to contact the patient but were unsuccessful OR Closed- Contact Made, Opted In if you were able to reach the patient and they participated in the survey, OR Closed Opted Out if you were able to reach the patient and they declined participating in the survey (should be rare) |
Open- First Attempt Made |
You attempted to contact the patient but were unsuccessful. When you go back to try on a different day and time, you will need to change the status to either: Closed- Contact Made, Opted In if you were able to reach the patient and they participated in the survey, OR Closed Opted Out if you were able to reach the patient and they declined participating in the survey OR Closed No Contact Made if you were unable to reach the patient after your second attempt. |
Open- Call Back Later |
You contacted the patient but they asked you to call back later. When you do call back at a later time you will need to change the status to either: Closed- Contact Made, Opted In if you were able to reach the patient and they participated in the survey, OR Closed Opted Out if you were able to reach the patient and they declined participating in the survey OR Closed No Contact Made if you were unable to reach the patient after your second attempt. |

You were able to complete the survey with the patient. |
Closed- Contact Made- Opted Out |
You contacted the patient but they did not wish to participate in the survey (should be rare) |
Closed- No Contact Made |
Your 2 attempts to contact the patient on two different days at two different times of day were unsuccessful. |
